To Mohez: thanks for the suggestion

Yes, Mohez, seems like that's where I need the lift. The orthotics I have now have no lift like the Birks -- actually, the lift is too far back on my orthotics. I'm going in next week to bring it to the docs attention. I find that the lift between the ball of my foot and before my arch (same place you mentioned?) is where I need the lift. I will try adding some pads this week to my orthotics before I go to the doc and see what happens. I was under the impression that the orthotics had to be molded to your foot and that meant no extra lift. But the lift is what feels good.

any luck with the orthotics with the metatarsal rise. If you get a chance
ask your doc, why this helps and if the rising of your metarsals actually
brings the strain on the plantar fascia ( at the insertion of the heel )
down thus giving it a chance to heal better/properly. Any idea how high
this rise should be ? Let me know if any answers
