Since you did not mention use of a night splint or strassburg sock I am wondering if your PF sets up for reinjury while you sleep. As you know, your foot tends to point down while you sleep - this allows your PF and tendons to contract - which in turn may cause reinjury with first step out of bed (a common syndrome). Have you tried a night splint? I'm not saying it's a cure-all, but it has helped many.
