New threading method makes more sense

I re-implemented the threading, but this time it is designed to make more sense. If you remember, it used to put new messages at the top above old messages in main index. It still does that. This makes it easy to see what's new. But it also used to do it inside each thread and sub-thread. That's the logical thing to do. But it really made figuring out who posted when and in what order really screwy. Now, the replies inside threads are inserted at the end of the thread. And replies inside each sub-thread are placed at the end of each sub-thread. You can see the test messages in the index for an example.
