pain in ball joint and going up

I have a friend who has pain between his first and second ball joint, occasionally radiating UP (definitely not down, he claims) to his knee or even his groin (where once upon a time he had a hernia operation). Changing shoes has helped but not eliminated the condition. He's been to several docs, none of whom had a definitive handle on the diagnosis or were of much help. Do you have any clue what it is he could have and what he could do about it? Thanks.

Could be a neuroma . This is very rare in the 1st or 2nd interspace.
