ESWT Questions

Posted by Mar on 5/25/02 at 18:08 (085214)

Had ESWT 4 days ago -- a couple of things I am wondering about -- i like to understand as much as possible -- 1) Why is ice the recommended treatment for plantar fasciitis but after ESWT we use heat - I understand we want to heal the fascia, but why didn;t we want to do that before? 2) my doctor said to use heat twice a day for 20 minutes -- would more often be even better, or is too much not good either 3) After being on my feet a little too much I am of course in agony, should I still use heat and not ice? Thanks -- Mar

We use ice for the first twenty four hours if painful then heat. Heat will
increase blood flow to the area and stimulate healing. Ice restrictes blood flow to the area.

In my practice we use heat. Ice is used if the heat doesn't help. Avoid too much ice Try heat first

Is more heat better -- like if I use it 3 or 4 times a day instead of twice? Mar

Mar, you ask a good question....Why ice, instead of heat BEFORE ESWT and vice versa after ?

What's the answer Doc's ?

Pete I am not a doc...but the heat increases inflammation of the inflamed tissue so ice before to decrease the inflammation...and after ESWT the tissue has been blasted by shock waves and broken up....heat increases circulation to the treated area...more blood flow...more and faster growth of GOOD tissue.

Ummmm...Dr. Z am I close??;o)
