1st Step Pain a requirement for Surgery

I posted a question on the Foot Surgery Board a while back but am still not clear on the responses.

When it comes to surgery is first step pain a requirment?

If yes, then why and is there another surgery suggested?.

If no, does lack of it reduce the probability of success?

I no longer have first step pain but have localized insertional pain.
I have had all available tests and treatments.

First step pain is not a requirement for surgery. We do prefer to exhaust all conservative treatments before proceeding with surgery though.
