I have worn custom inserts with no metatarsal support in them for two years. I just tried a new insert that has some metatarsal support. It almost feels like like a rock under my foot. Is this normal. Should you feel metatarsal support? Will it be uncomfortable at first?

If placed correctly, the met support will feel uncomfortable for awhile. I remember trying it and it took me a couple of days to really get used to it.

What is correctly? It felt like it was slightly behind the ball of my foot. In the middle.

Yes. It should be placed just proximal to the met heads. It feels very weird, but you should get used to it soon.
