Patient history form to take to the Dr.

I am thinking about doing a one-page patient 'history, symptoms, and treatments-tried' form for patients to fill out, print, and take to their next doctor appointment. Do our doctor's have a list of questions they want answered for the typical plantar and heel pain patient?

Off the top of my head
Do you limp or have pain when you first get up in the morning or after sitting for any lenght of time
Duration of heel pain
Has it been getting worse
What activities can you do since your heel pain
Is your heel pain effecting your lower back, hip, knees
Has this pain limited your activities, job work.
Did you injury your foot
Any previous history of heel pain
What worked in the past
Have you had local steriod injections before. In any other joint, tendon. Did they help you?
Any change in excercise, shoe gear
Weight gain
What do you think caused your heel pain
What have you tried so far. What has helped
What has made this worse
Have you seen a foot and ankle podiatrist or orthopedic doctors
