ESWT

Posted by Elyse B on 7/04/05 at 09:26 (177788)

good luck to all of you who try ESWT.

It would be greatly appreciated to all on this board if those who have had ESTW post your successes and failures. To Elvis, good luck on your ESWT and may you have happy running days, we all await eagerly your progess since you are huge proponent of ESWT. When are you having it?

This board has been very informative and I have learned a lot.

Again good luck to all.

the company I deal with will attempt to appeal with the carrier and not charge the patient for their component, may be worth looking for provider with them

I'm not a big proponent of ESWT. I've been suffering from pf for 18 months with limited success and pf is now a 'no-brainer' and well worth the $500 treatment fee. I haven't decided when to get it but you'll be about the 50th person to know. Good luck.

DavidW on 7/04/05 at 17:21 (177806)

Elyse B. I had ESWT in January with no success at all. It was a last hope since I have tried everything else possible. Cost me around $600 out of pocket. Cost my insurance carrier $4000. Treatment was high energy, only one foot. I think, like orthotics and everything else, that ESWT was recommended because my doctors (yes, several) had no other ideas.

David,
Luckily you had some insurance coverage. I'm sorry you got no improvment from your treatment, but perhaps down the road a few more months you'll begin to show some improvement.

Your case and others like yours are what Elyse and I are concerned about and what causes us to doubt the positiveness reported about ESWT regardless of the studies posted here.

Currently insurance coverage is pretty sparse but what happens when there is insurance coverage with more doctors doing the treatments and there are still the same type of poor results? Patients lose in many ways, but doctors have the potiental to make a great deal of income in spite of patients losses as long as they can perform ESWT both on label and off.

I'm sorry to say that sometimes I begin to think it's a sham treatment and I don't really want to see it as such. I want to see it as a cure, and wish I worked in an area where I saw this happening each and every day then I think I'd feel better about ESWT for P.F. I guess I'm like Thomas I need to see it with my own eyes to really believe it.

Re: ESWT

Dr. Z on 7/04/05 at 18:39 (177809)

Why would anyone recommend a treatment when they have no idea of what the problem is.
Ralph you could place any procedure, in your paragraph about poor results.
Are you in need of ESWT Ralph? If you are? contact a local doctor interview, watch an ESWT procedure. ISn't that what a good consumer would do?

Common Sense should be added to a pf treatment plan. There are specific indications, inclusion and exlusion that go with ESWT treatment. Patients lose in many ways. When a procedure fails most doctors I know feel the lost themselves that is what makes them a professional and a member of the health arts

There is still a second treatment option or you can move to foot surgery

I make a lot more money on foot surgery than on ESWT so if anyone prefers the surgery, let me know.

I will keep that in mind

Re: ESWT

DavidW on 7/05/05 at 08:18 (177837)

I have had 3 years of constant pain that has not responded to ANY treatment, ESWT was just one of them. I just feel that all the doctors I have seen did not know what to do, so ESWT was recommended. I don't think it was a bad recommendation for someone with my pain; I probably would have done the same (and I did ask for it). Most doctors were reluctant to recommend ESWT without many months of conservative treatment, which I think is good and fair. I do however have issues with the treatment's success / failure ratios because no one, not the doctor nor the treatment center, has followed up with me as to my progress. How do they know if it was successful or not? Also, the treatment center stated that they would not repeat the process for at least 1 year since they considered the treatment a 'cure'.

I have however, for the last 6 weeks, been seeing a licensed massage therapist. I have had 9 painfull sessions so far, and have had stelar results. This past weekend I went to the park, food shopping, and played outside for many hours with almost no pain (15 minutes has been my limit up until now). I am not sure what the heck I actually have or why I have had so much pain. I also am quite perturbed by how painful and deep massage can actually be helping me so much. I have 6 more appointments scheduled and, for the first time, feel that I am actually going to be pain free at last.

Dr. Z,
My P.F. pain is located on the opposite side of my feet. On the outside and down the entire length of tendon in my left foot. I have some arch pain, but no first step morning pain at all. How can ESWT help me?

Re: ESWT

Dr. Zuckerman on 7/05/05 at 09:57 (177852)

Ralph,

It can't help you. It appears that you have lateral band plantar fasciitis and or the peroneal tendon longus and or brevis. A gait analysis and biomechanical examination could help you

Re: ESWT

Ralph on 7/05/05 at 10:04 (177853)

David,
I was told but I don't know if it's true that the Fascia in the foot is made up of many different layers. Under healthy circumstances these layers slide over one an other smoothly. There is no inflamation, no pain.
When someone gets a case of P.F. the various layers of fascia maybe it's called tissues become inflamed and cannot slide smoothly
across each other do to a variety of changes. Inflammation just be one of them. This friction causes pain.

Perhaps your massage therapist is helping you to get rid of inflammation and realigning the fiber layers if that is possible of your P.F. so they slide smoothly again.

I'm not a doctor so please don't take this as medically accurate. It's just something that I was told by a Physical Therapist. It may be just her way of explaining P.F.

Re: ESWT

Ralph on 7/05/05 at 10:32 (177857)

Is lateral band P.F. treated the same as regular P.F.?

Re: ESWT

ESWT Man on 7/05/05 at 12:49 (177869)

Hi all. I can vouch for the success of ESWT first hand. I was a sales rep and tech for Healthtronics for over three years. I performed hundreds of procedures and I was getting close to 90% success which is higher than any of the studies. There are some MAJOR changes going on now though. Medicare is coming out with new fee schedules very soon and there are several new devices out. The high/low energy issue is about to become CRITICAL in determining how this is reimbursed. The rates might be determined by high vs. low energy, and in office vs. facility. The BIG question is how is Medicare going to define 'high energy'. Should be interesting.

Ed Davis, DPM on 7/06/05 at 00:55 (177899)

DavidW:
You may want to consider sonogaphy or an MRI to be a bit more certain of what exactly you are dealing with. I tend to like to know more about what I am dealing with before starting treatments unless things are very obvious.

Re: ESWT

DavidW on 7/06/05 at 06:30 (177904)

I have had two MRIs, several X-Rays, and a Bone Scan. All showed very little if anything, yet I have been unable to be on my feet for more than a few minutes for years. The Bone Scan and MRI results both stated the 'possibility of a very mild plantar fasciitis'. I have seen three podiatrists, blood doctor, internist, neurologist, orthopedist, had five weeks of physical therapy, custom orthotics, pain management doctors, ESWT - you name it. I am very familiar with 'The Book' and taped my feet daily and iced several times a day for over 1-1/2 years. Absolutely no results from any of the above.

I had ESWT done in January of this year. My doctor states that best results from 5-6 weeks and after that it is a bust. The treatment center says results may come up to 3-4 months. At the 5 month mark, I had 0% pain relief. I do not think that I have PF.

Anyhow, I like Ralph's description because it makes sense. Maybe I had PF, but with all the inactivity for 3 years and all the icing, I may have created a different cronic problem. All I know is that the deep massage is working. This morning I walked .90 miles with almost not discomfort (yes, .90 miles is a weird number, but I am using Scott's advice of starting slow and adding 10% each month). Two months ago, a .25 mile walk would have left me in pain for several days.

I think there is a certain number of people out there that have symptoms similar to me and also are getting no results from all the available treatments. I think that they should try alternative solutions outside of standard medical practice (like deep massage, TFM, ART or whatever other things are available). I, like John from MN, am having astounding results.

Does Shockwave Therapy useful for nerve conditions such as RSD or Neuropathy? Will it help to decrease the pain of these conditions? Suzy

Wouldn't a link to your new organizations website be sufficient. These types of posts reminds me of the preacher with tooooo long a sermon. It's a turn off. Too much to absorb so you just automatically turn him off.

Re: ESWT

DavidW on 7/06/05 at 07:37 (177908)

Also, I have had pain in both feet for three years. I know that the massage is working because both feet fee better. I only had ESWT on my left foot.

Re: ESWT

Ed Davis, DPM on 7/06/05 at 11:16 (177912)

David:

You did not mention an NCV. Many NCV technicians don't have the skill to make the diagnosis but two diagmoses to consider: medial calcaneal nerve entrapment and entrapment of the first branch off the lateral plantar nerve AKA Baxter's nerve.

The only physiatrist with sufficient skill to make such a specific diagnosis via NCV is Dr. Mohammed Saeed of Tacoma, WA.

Ralph:

Some readers like it and some don't. I am not here to judge. Dr. Rompe, who posts here irregularly decries the lack of scientific information on this site.

One has the option not to read posts they don't like. The doctors, on the other hand, need to read most, when they have time because they are often asked for opinions.

Dr. Ed,
I just saying when information such as this is jammed together it's passed over and is probably merely taking up space. You do what you want. I personally think most doctors looking for research look to other sites for their information. I think they have many better resources available to them for this type of information should they desire it some of which no doubt they already pay to have access to.

Elvis,
After that post falls off the page how is anyone ever going to find it again or even know that it existed here? The life of that list on this website if very short. Anyone who wants it should print it out now.

I discovered a natural supplement called XANGO which is made from the mangosteen fruit. I have Inflamation in my heals (plantar facitiss) and had trouble everyday getting out of bed to walk. I was taking vioxx everyday and even had a couple of cortizone shots and even then, I still had days with pain. I started taking XANGO and within 10 days, I was and still am pain free. I believe in the product so much, I am a distributor. Check out my website http://www.np.natureswellnesssecret.com or email my at (email removed) with any questions. I know first hand what type of pain everyone is dealing with. Wouldn't you want a natural alternative to offset the pain? I do and am taking that approach right now.

Dr. Ed Davis, I had an NCV done by a Neurologist with negative results (left side of body from shoulder to toe). I also had a visit with Dr. Z and he did several nerve tests on each foot with a small handheld device. He also stated that I had no obvious nerve issues. It is amazing that the body can create so much pain yet there is physically nothing obviously or detectably wrong.

I do appreciate all the feedback from everyone...thanks.

After my .89 mile walk yesterday, I still feel great!!!!

I had this procedure ....wonder how long it takes to recuperate and go back to work if you are on concrete with heavy lifting and a very physical job. Had EWST three weeks ago, doctor sees minor changes. I am on short term disability and wonder whats next for me.
