Diffuse PF pain vs. acute pain at heel

I am a 25 yr. old mail who has been suffering with PF for 2 1/2 years now. My PF pain is not the "classic" pain focused at the heel, but is more diffuse -- focused mostly in the arch, I would guess. Can anyone talk about how therapies might differ based on these two scenarios? This would include surgery, cortizone shots, etc.? Of everything I have done, massage has given me some tempory relief, but it comes right back.
