OssaTron "double blind"

How can a study be "double blind" when the real treatment causes pain? The patient will know he's getting something and the doctor can see who is getting a painful treatment and who is not, thereby biasing the results, especially if the doctor profits from future application of the treatment. Biased towards "proving" the treatment helps. I wonder why FDA approved this.
